PERSONAL DATA PROTECTION INFORMATION AND CONSENT SHEET

PharmaSoft Ilac Bilisim Danismanlik Hiz. Ltd. Sti. attaches importance to the privacy of personal data and privacy in the context of its corporate privacy policy. In accordance with the Turkish Personal Data Protection Law no. 6698, the all personal data you shared with our company or obtained by our company through automated or non-automated means will be processed by our company in the manner specified in the Law and in the framework set out below and in the manner described below with the title of Data Processor and we also declare that any transaction carried out on your information, including disclosure, transfer and availability, shall mean the processing of personal data.

“Personal Data” means any information that identifies or determinable your identity.

PURPOSES AND LEGAL REASONS FOR THE PROCESSING OF PERSONAL DATA

Our company, which is certified by the Turkish Ministry of Health in accordance with the Regulation on the Safety of Medicinal Products, serves as a Contracted Pharmacovigilance Service Organization to give pharmacovigilance services to pharmaceutical companies.

Within the scope of Pharmacovigilance services; adverse reaction reportings can be gathered either through our Pharmacovigilance database or it can be gathered orally through our call center. Your personal data are processed by our company under the Turkish Personal Data Protection Law no. 6698 for purposes such as fulfilling obligations and commitments (i.e. pharmacovigilance data storage, adverse reaction reporting and informing the national Competent Authority) in accordance with the contracts to which PharmaSoft is a party and also for the fulfillment of obligations and commitments within the scope of Pharmacovigilance services that are provided by our company.

DATA TRANSFER AND PURPOSES OF THE TRANSFER

Your collected personal data is subject to the personal data processing requirements and purposes set out in Articles 8 and 9 of the Law; data may be shared with third parties under the confidentiality agreement for the purposes of data processing, data management or technical support. Personal data may be disclosed to third parties for the purpose of supporting criminal or other legal inquiries or prosecutions in or out of the country under applicable law, court decisions or decisions taken by public institutions.

RIGHTS OF PERSONAL DATA SUBJECT

As personal data subject you have the right to;

• Learn whether or not her/his personal data have been processed;
• Request information as to processing if her/his data have been processed;
• Learn the purpose of processing of the personal data and whether data are used in accordance with their purpose;
• Know the third parties in the country or abroad to whom personal data have been transferred;
• Request rectification in case personal data are processed incompletely or inaccurately and request that the process performed in this context be notified to the third party to whom the personal data is transferred,
• Object to occurrence of any result that is to her/his detriment by means of analysis of personal data exclusively through automated systems;
• We would like to state you that you are entitled to claim damages in the event of a criminal offense as personal data is processed in violation of the
• Data subjects will be able to forward their requests for their rights listed in Article 11 of the Law 6698 to PharmaSoft via info@pharmasoft.com.tr or other methods determined by the Personal Data Protection Board. PharmaSoft will evaluate the requests within 30 (thirty) days at the latest.
